Spa membership

payment

• Memberships are paid by direct debit on a day requested by the client. New members joining during the month, who want to use benefits that same month, must pay for the first month at the time of joining.
• Joining Fee: $10 one off payment to pay on joining.
• Payments will be handled by EzyPay for monthly membership fees.
• Client also completes application form for EzyPay and signs Membership Agreement.
• Members are required to notify the spa in writing of any change of bank account information on file prior to the next electronic funds transfer (EFT) date. Payment Authorisation Forms are available at the spa.
• Payments will be due on the 1st of the month (or next business day) or members can request alternative day for direct debit.
• If a payment is dishonoured for any reason a charge of $5.79 will be payable in each instance the payment is declined and will be debited from the members account.

general

• endota spa memberships are not transferable and can only be used by the person purchasing the membership.
• Unused membership treatments may be carried forward.
• Memberships are only able to be redeemed at New Zealand endota spa locations.
• Members can purchase more than on membership at a time.
• Term of membership: 12-month contract with auto-renewal.
• You may cancel your membership, after fulfilling the 3 month commitment period, with a 30-day advance written notice to endota.
• Payments for upgrades, other treatments or product purchases are payable at the spa at the time of purchase.
• 24-hour cancellation notice for treatments are required or service may be forfeited.
• endota spa reserves the right to change rules, regulations and pricing, or withdraw the membership program at any time upon providing reasonable notice.
• Members agree to follow the spa rules and regulations. endota has the right to refuse or discontinue service for any reason.
• Adolescents from the age of 12 to 17 are welcome to enjoy a selection of treatments with the permission and supervision of a parent, guardian or caregiver, who must also sign the client card prior to receiving any of the membership services.
• endota does not perform treatments on children under the age of 12.

freezing your membership

• There may be times when members would like to freeze their membership due to extenuating circumstances, such as extended illness.
• A minimum of one membership payment must have been made before a membership is eligible to be frozen.
• Members may freeze their membership for a minimum of 30 days and a maximum of half of the original membership term.
• Memberships may be unfrozen at any time at least 30 days after the start of the freeze or will automatically recommence on the pre-determined freeze end date. At the end of the membership freeze term, the dues will continue with the next scheduled electronic funds transfer (EFT) date.
• A membership must be unfrozen a minimum of 30 days before it can be frozen again.
• The membership expiration date will be extended by the amount of time that the membership was frozen from. Terms, conditions, rules and regulations of the original Membership Agreement continue to apply through the extended membership term. To initiate a freeze, simply complete the Membership Freeze Request Form at your spa.
